Migración del catéter de derivación ventriculoperitoneal con protrusión por cavidad oral

Resumen: A pesar de que la derivación ventriculoperitoneal (DVP) es considerada como el manejo definitivo de la hidrocefalia posthemorrágica en la mayoría de los casos, sus complicaciones aún son comunes, con un 25% correspondiente a aquellas de origen abdominal, dentro de las cuales se encuentran la perforación de vísceras abdominales y la migración distal del catéter peritoneal con salida por diferentes orificios naturales. La protrusión oral del catéter de DVP es una complicación bastante rara y desde el primer caso presentado en 1987 se han reportado solo 22 casos a nivel mundial y ninguno en Colombia. Debido a sus potenciales complicaciones como peritonitis, meningitis o ventriculitis con importante morbimortalidad, se requiere un tratamiento oportuno. Sin embargo éste no se encuentra bien establecido dado lo inusual de la condición.
Por este motivo presentamos a continuación el caso de un lactante mayor con protrusión oral del catéter de DVP, junto con una revisión de la literatura
analizando los principales factores relacionados con dicha condición, su patogenia, complicaciones y las posibilidades de manejo, proponiendo un tratamiento
sencillo, efectivo y seguro para el paciente, con menor riesgo de complicaciones.
